Forms and Payments

Below are the Membership Registration Form and the Event Activities Form. You will only need to fill out the Membership Form and turn it in, along with your dues, to become a member.

The Event Activities Form is designed to help our parents who have volunteered to coordinate an event: Mom's Night Out, a field trip, an educational share, etc. It gives those volunteers a template to help know what to include in the description of the event. 

 

Online Payments:

We gladly accept cash, checks and money orders as a way to pay membership dues.

We have attempted to make online payments an option for PHSA members. Due to IRS requirements for non-profit organizations, such as ours, the board decided not to pursue it. We apologize for any inconvenience this might cause you.
